Instructions
- Call to set up an appointment, and have your insurance information ready (this is always asked)
- All the appointments are Virtual only
- Fill up all the paperwork, with the best of your knowledge
- Submit the paperwork thru our email with a copy of your insurance, picture ID and list of all you medications including OTC
- The therapist will contact you via Zoom or phone call at the time of your appointment
